I think the only option is to call the doctor who wrote it as a witness to give evidence in court about what it says

If the doctor is not available a chemist who was used to reading the doctor's prescriptions may be able to help

If it is important for a court case, get the doctor to type up a proper report.

Once you get an intelligible form of the statement your lawyers can ask the other side to agree the contents of the report.
This dispenses with the need to call the doctor to give evidence.

However, if the other side are disputing the substance of the doctor's report he will have to be called as a witness.
