PayPal Form - Helpful Tips

    I've been asked to create a popup for my company (charity company) with a donation form on it. I don't have an issue with the popup but I'd love some help with the form.

    The idea is that when you go on the website that the form would popup (image link below). The person giving a donation would pick either One-Off or Monthly Payment and then pick an amount underneath that and press PayPal. Any ideas on how to do this?
