At what point when developing a web db app would you need to start looking at transactions?

you could write pages on this but at a minimum
you should have your save/update to the database in a transaction

Does anyone here who writes shopping carts use transaction in MySQL? I understand (from webmonkey) that you have to use a different type of table.
