Minimum charge/ Setup Fee

Could anyone clarify which companies charge Setup fees and which charge a Minimum Charge on calls.. I know that Eircom have a minimum charge and UTV/Esat have a setup fee, but what about NTL, Cinergi & Chorus CPS? Can't seem to find anything on their websites.. Thanks, David.
