Feel free to move as i didnt know where to post it. But anyways i need the above for the interview for my visa to work legally in the United states but i havent the foggiest where i can get it.

If you're applying for a visa I assume you'll have to get a medical. Get them to certify you then. They can probably advice where to get them. I know if you get 'shots' for going travelling to high risk areas they usually give you a little booklet stating what and when they gave it you .
