Hmm the most notorious gaming glitch is good, i'd add the wow disease to that. Although it's not exactly a glitch, more of an oversight by blizzard, but that;s what most glitches are I suppose.

Most notorious glitch would probably be the end of Pacman where there's a split screen and the game is impossible to get past.
