Sky+ failed recordings

I'm getting a lot of failed recordings with no reason given in the Sky+ planner. I've got series linked recordings set for 3 programmes a week - "Lost" on RTE2 Mondays, and "Extras" plus "Catherine Tate" on BBC2 Thursdays.

Nearly every week, at least one of them fails  for "unknown reason".
And this week, I checked to see if Lost had recorded correctly because it was on early and was a double episode, and it looked OK. It wasn't till last night we sat down to watch it and now it's saying "Recording failed" What's going on

We've never gone below 20% space free. However, that's another problem - I'm convinced these failed recordings are not freeing up their used disk space correctly. If I add up all the OK recordings I've kept, they come to 60%, but the planner is telling me I've only 23% free space! I've tried the disk reorganiser thing in Installer's menu, but this doesn't help.....

    Any ideas?

I've had a search, but didn't solve my issues.
Running the Planner housekeeping again has freed up some space, but I still can't work out why a recording of Lost on Monday which worked OK then changed to Failed later in the week

Anyway, Lost was repeated on Friday night, so I set it to record again. Checking it next morning, the planner entry had completely disappeared! No failed message or anything, it simply didn't appear in the planner.

    What's going on?

Yep I’ve got that, programs that worked fine just come up as a ‘failed’ for no reason a few days later, links disappearing and even the odd program going missing that was recorded but not viewed. Yep it’s a pain in the ass but it’s only a few times a year I have to deal with it.
