Lawn help - bad thatch patches

Hi all, my back lawn is very yellow this autumn. A combination of leatherjackets damage, too much fertilizer, hot weather and thatch grass has meant my lawn has never recovered this year. Typically my lawn is green and decent grass cover. Any ideas how to repair/improve before the winter sets in?
