DIY Divorce process, info needed please

    the case is straight forward, we are eligible and by consent. I understand the process works like this:
    -Applicant submits the Civil Bill: I file the Family Law Civil Bill in the Circuit Court in triplicate

    • Respondent confirms receipt: The spouse (respondent) confirms they received and consent to the application; if not attending the hearing, they do so by affidavit.- I am not able to get anywhere a sample of this, can someone please help?
    • Affidavit of Means (Form 37A): Both spouses each file a sworn statement detailing assets, income, debts, liabilities, and outgoings.
    • Affidavit of Welfare (Form 37B): Not applicable in my case
    • Mediation advice (Form 37D): Not applicable in my case
    • Notice of Motion + Affidavit: When in full agreement, file a Notice of Motion and accompanying affidavit confirming compliance with divorce rules and outlining the final agreed terms -I am not able to get anywhere a sample of this, can someone please help?
