Carers Benefit - "A statement from accountant if you are self-employed"

    Looking at the application for Carer's Benefit for Self Employed Persons and one of the Check list items to be included as part of the Application for Carers Benefit is 'A statement from accountant if you are self-employed', Would anyone know what this 'Statement ' would be , is it a Form 11 or something else. I rang Social Welfare and they were not too sure , would anyone know what this 'Statement' would refer to.
