Health insurance for teens

We never had health insurance for the kids. What do people do when kids hit 16 and are no longer eligible for childrens hospital? Do you put them on a private plan at that point? (PS - is there any active parenting forum in Ireland? This one seems very inactive, but I dont know of any others_).
