Second/redo root canal on same tooth or extraction?

    ABout 3 years ago I had a root canal on the back lower left molar with a crown the tooth was dead.


    Recently it started to ache and there was a little swelling I was told there was an abscess inbetween the roots or that area looking at the ex ray by a general dentist.


    I was told to go for extraction rather than try a second root canal. I was told the area of the infection was unusual and no explanation as to why it happened.


    I have heard that after tooth extraction there is dental drift and your teeth move also that there is bone loss in the jaw and this can age your appearance. I am concerned about that. I asked my general dentist about this and he said no that doesnt happen?


    Also it was my general dentist who referred me to get it extracted as he worried some of the root would be left behind as it might be brittle if he did it himself.



    I wondered is a second root canal preferrable or possible.


    What are the dangers of dental drift or bone loss with extraction for back left lower molar right at the back.


    Also getting an orthodontest apt for a consultation seems impossible with no dates until feb. I dont think its safe to let it go that long the infection could spead to other teeth or tissues.


    I have a referral to get it just extracted. But I wished to see if second root canal is poss.


    My dentist just said it will keep coming back (the infection) even with second root canal. He is just a general dentist though.


    My general dentist never recomended having an orthodontist look at it first before extraction.


    I have to say I dont have a good opinion of this general dentist. As every filling he has done for me has come off in a week. And there are some other issues. But they made me go to him as he is the senior general dentist.

    What are risks for extraction?? It feels so final?? Bone loss ? risks of extraction to other teeth?? dental drift?? I asked about these and my dentist said no?? why?? He did say i had plenty of bone there on the lower jaw.


    Also with the time of year it seems i cant get an orthodontist for EXAMINATION until FEBUARY ! Which seems dangerous to leave infection so long. ( i have just had antibiotics). But i might be able to get an extraction before then but it would have to be without a consultation from an orthodontist. Just bring the referral straight in.


    My dentist says right now the infection is lying dormant but would just return?? Its still a bit tender. And says even with second root canal it would just come back.


    But he refusses to admit extraction has risks like dental drift and bone loss and a risk to other teeth having to be extracted. He flat out said this would not happen. Which I find hard to believe and felt he wasn't great for giving info.


    Is it worth getting an orthodontist consultation for seeing if a second canal can be done or just go straight for extraction?? I only ask because given the time of year I WILL HAVE TO WAIT WITH THIS TOOTH UNTIL FEB FOR GETTING IT EXAMINTED BY ORTHO. That seems dangerous.


    Should I just get the extraction?


    I just dont want an aged appearance. He suggested an implant if i worried about chewing ( i chew mostly with my back teeth) thoughts?


    Also how much are implants for the very back molar? And was my dentist being truthful in saying there will be no risk of dental drift and bone loss as I specifically asked about this to him? But he is just a general dentist.


    Also why does my dentist say there will be no drift with the extraction ..i know there will be no gap because its the VERY back one but still won't my teeth shift? He said no this wont happen? And he also said there would be no bone loss but how can he say that??? I kept asking and he was reasurring me saying nope nothing will change?

    Should my dentist (general ) not have referred me to orthodontist for consultation (to see if root canal was possible) first before i was referred for specialist extraction?

    Does reinfection always come back after second root canal?


    The first one was successful by the way but only for 3 years. And I got a crown.


    Also why did it get infected between the roots? my dentist said it was very weird and unlucky.


    Am I better off just getting it out and getting an implant? Or just trying a root canal. My general dentist said it would just fail. He did say i was a good candidate for a implant tho as i had lots of bone. And my root canal last time was several thousand with the crown. So maybe he doesnt want to waste my money?


    Also I think when I had the root canal done the dentist said the tooth wasnt great. vaguely i think he said something like he hoped i would get five years.



    Its the only tooth i have issues with!


    So should I wait till feb to get orthodontist apt for consultation or just go for specialist extraction as soon as possible. And what are risks with extraction for REAL. ?? I feel my dentist is bsing me saying there will be NO DENTAL drift whatsoever. I have heard your bite can become misaligned. And other teeth damaged. There will be no gap between teeth as its the furthest back one but i find it hard to believe that there will be no drift.


    My dentist just seemed uneducated. If he were not forced upon me by the practise i wouldnt be going to him. Just being honest. In fairness tho another dentist at the same practise said it would just come back again anyway.


    We didnt go into risks with dental implants like pain etc which i have heard.


    Also he didnt mention bridgework ...but i hear that extractions and bridgework go hand in hand? Even tho there is no gap BETWEEN teeth here in my case ... there will be more space in my mouth?

    Only other teeth i had extracted were all wisdom teeth as there was no room. Also one baby tooth as it refused to fall out. Never had braces teeth are straight and i want to keep them that way!


    also he didnt ask me to say anything specific to the dentist doing the extraction as in how to prepare for implant. Only thing he said was i had plenty of bone NOW OBV. I feel he is either not informed about risks for extraction or just cant be bothered to say ..or i am an extremely rare individual. He is 'OLDER' so .. maybe he doesnt see the point of trying to keep it and is more used to extractions' The practise made me go to him as he is the 'senior dentist'. Usually i can avoid going to him at this practise but they made me because it was an abscess in the gum. The other dentists are better.


    He didnt even go through the changes my mouth would go through after extraction ... he was like no ..no changes whatsoever. He actually said that.

    I stopped going to him at this practise after two minor fillings he did came out the day after him doing them on two sep occassions.


    Surely extraction will cause bone loss and other teeth to become loose? he said no ..but it doesnt make sense?? Why does he think a second root canal should not be attempted or even sending me to an orthodontist or endodontist to even see if its possible first??

    He seemed to say this will save you money. I trust his has the best intentions my sixth sense tells me he is trying to do his best. But also my sixth sense tells me he should have have told me about things like dental drift. As I asked will my other teeth be affected or move etc will i lose bone?? He outright said no. Just said you have plenty of bone there. Well Yeah NOW.


    Yes also there was an ex-ray .. it just showed the black dark area of infection which did shrink to almost nothing with antibiotics but my dentist said it would come back. The infection was between the roots. And he had no explanation as to why it happened. The crown etc is fine no movement. swelling has gone down
