Travelling in a few months for a holiday. My previous ESTA is expired, do I simply need to do a new application or is there any extra stuff since Covid?

Just the new ESTA application. The US has dropped the vaccine requirement for entry.

Just complete a new application. Make sure you only complete it on the official website which is https://esta.cbp.dhs.gov/ as there are loads of dodge sites around and charging more .
