S1 Form - Healthcare access while living abroad

    I'm travelling to Germany and will stay for an extended period, maybe 12-18 months.

    This is not for work purposes.


    While in Germany I want to be covered by the German healthcare system.

    Since I plan to reside there for longer than 3 months, the EHIC card won't be of any use to me. EHIC is for short trips only.

    I would join their statutory public health system if necessary.


    There is an S1 Form (Certificate of entitlement to healthcare) which contains details of my Irish healthcare rights.

    Presumably the form has my PRSI contributions.

    So I would be using my Irish health insurance record as proof to the German authorities that I have a history of being in a EU public health system and am insured in Ireland.


    Does anyone know how I can apply for this form?

    Is it from the HSE?
