Is the image taken from inside or outside?

Looks like could be a poor installation (same as mine) with possibly some small gaps. If it's windy can you feel any slight breeze around the colder areas?

It has been taken from inside. No gaps around the door, seal is quite good.

And where the frame meets the wall/floor? Looks like there is something going on with that very cold patch on the middle left?

If there are no air gaps then convection isn't the problem and it would be conduction from the cold floor/ground below.

It might be "A-rated" but that's against a subset of hall doors, meaning that it's probably the best of the bunch. But that does not mean that you won't see heat-loss - at the end of the day it's only 71mm thick and likely filled with ~55mm of polyiso insulation - that doesn't make it impenetrable to cold!

Also be cautious of how you interpret the data from the FLIR - you really need consider the internal temperature, external temperature and other objects within the field of view. You can't just point it around like a blunt object and declare defects on the basis of the screen colour.

On the flip-side - if that door was made from aerogel then I'd be wanting my money back.

As for the frame issue - that looks minor enough and bay be a small building defect - I wouldn't necessarily be kicking up a stink.
