Have a quick question, my employer and employee pension contrubition is around 18% combined. I max mine to make the employer max their %. I also make a 2% AVCs. Based on the age % Relief attached if I combined emplloyer, employee and AVC I am above the % threshold for my age. Do the below %s only apply to personal contrubitions and AVCs?

    https://www.revenue.ie/en/jobs-and-pensions/pensions/tax-relief-for-pension-contributions.aspx

I have never applied for Relief with Revenue as I think my employer does it but I presure I should be applying for the AVC releif?

It's only the EE contributions that are relevant in arriving at the max % for age. Unless the plan is a PRSA in which case the ER and ER contributions count towards the max % for age.

Normally EE regular and AVC contributions are tax relieved on the "net pay" arrangement via the ER so tax relief is granted already on your payslip by reducing taxable pay by the amounts contributed. You may wish to confirm that though with HR to be sure that both regular contributions and AVCs are both treated in this way in your case. If any allowable contributions are note dealt with on "net pay" arrangement you can submit on a review in your Form 12 to Revenue.
