    Hope I’m posting this in the right place.

    My grandmother has early stage dementia and my grandad wants to put things in place in the event she lives longer than him or he becomes unwell. From a health perspective it would be mainly that doctors treating her have permission to discuss her care with another family member. Is there a process you need to follow to put something like that in place where the patient no longer has capacity to give consent?
