Cistern filling slowly

I have one of those push button toilets with two buttons which has been working fine for years but over the last few days has started acting up. It's filling extremely slowly and when I opened the lid (as per attached photo) I could see at the location marked A a high pressure jet of water was coming out in the direction D. Raising or lowering the arm (B) opened and shut the valve that controlled this jet of water but didn't increase the refill speed.

Behind location A on the photo you can see a twist cap, I've opened this and there's a sudden influx of water but tightening it again just reverts to the jet of water instead of a proper refill amount. Removing the arm that connects A to B basically removes the ability to open / close this valve but again doesn't seem to help with the speed of refill.

This has been going on a few days now, one time I opened that twist cap behind A and for whatever reason it all started working again, refilling as quickly as you'd expect. However, a day or so later is reverted to a slow high pressure jet of water and opening / closing that cap didn't make any difference the second time.

The point C on the photo doesn't appear to do anything but I can pop it off so I've included it in case it means anything.

Any ideas what I can do to fix this?

Replace the filler - hopefully you have a flexihose with a valve on it connecting to the underneath of the cistern to isolate, if not get one fitted? Handy for leaks etc. You can get a whole kit that includes the top button / drain. Sorry, don't know what's up with the old one....
