Health Insurance on Form 11

My husband's employer paid health insurance for 2020 for him, me and 3 kids.

There is a monthly BIK figure on his payslips. There is no deduction as all incurred by the employer.

Should this go on the Form 11 under Medical Insurance Relief with the gross premium for each family member?

If the employer pays all then your husband pays BiK on this as you say. However you are entitled (subject to the max amounts on the link above ) to relief on the premium for each. That's potentially worth up to €700 in tax ( max 200 per adult and 100 per child ). So enter each person on the F11, tick if child, the premium , the amount paid by your husband ( which is 0) and the number of months in force ( usually 12) and you'll see the relief in the computation page of the F11.

You can go back up to 4 years if this occurred in earlier years also.
