If you are on long-term JSA and receive a JSA application form to complete with note attached 'for our records concerning date XYZ', what does this mean? No answer at SW office.

At a guess they are seeking to clarify if you were working on xyz dates , albeit I don't understand why you received an application form for a benefit your already on 🤔
