Online passport application... COVER PAGE??

    Hi
    Am in the midst of an online application for a minor and have to send back the consent form which is fine however I also need to send back a "cover page"... Anybody know what this is??

    I don’t have it to hand now but there’s a separate sheet from the consent form with “cover page” written on it , and it has the application number , qr code and i think also the address of the passport office where you send it all
    There’s another page again with a checklist, but you don’t send that
