    I think the name of a particular form is called Inland Revenue?

    It must be a historical name / usage.

    The Irish revenue is officially The Office of the Revenue Commissioners.
    UK Revenue is HMRC ,Her Majesty's Revenue and Customs.

    The Inland Revenue Affidavit (Form CA24) was the name of the particular form.
