Got braces a month ago and had the first adjustment on tuesday,

One of the braces came loose that evening so went back on wednesday to get it refixed.

Same braces has come loose today and will call to get to redone.

anyone know why this happened and what should i say to orthodontist when i go back again ?

Depends on the type of brace, the bite, habits& food, bonding agent used, whether it's tooth or artificial tooth that is being bonded to, ect ect.
Regardless, twice in a week is too much for both you & the orthodontist. They'll be just as keen as you to avoid unnecessary repeat visits.

Thanks for reply , there the tooth coloured braces (not Invisalign) . Don't think I ate anything wrong had all soft foods and it's on a real tooth . It's the no2 on the bottom and it's recessed a good bit so that's maybe why it keeps coming loose 😢 I can feel a little pop then it's loose . Hopefully it'll get sorted tomorrow anyway
