Maternity leave.

    Hi, I was hoping for some guidance. I am due to start my maternity leave monday coming... but i got signed off a week early due to pregnancy related illness. My gp gave me a sick note. However i was chatting to a colleague who handles our hr dept and she mentioned i need a social welfare form from my gp for every week before i give birth. Does anyone know the name of the form? I am entitled to full payment whilst on maternity leave for 26 weeks. Apologies if this has been asked before.

    The GPs secretary will be more than familiar with the form.

    I don't know it off hand myself, but I had to get it too, I'd just collect the signed form from the secretary each week and send it into work.

    You just need them until your maternity leave kicks in.

    You don’t need it for every week before you give birth, just for the week before your maternity leave / maternity benefit starts.

    You will be on maternity leave and benefit 2 weeks exactly before your due date. You will need a sick cert for any period you are out sick up until your maternity leave starts.
    I don’t know how people get jobs in HR when they don’t even know the basics.

    So you can claim I assume, Ib1, illness benefit form.
