Actors reading lines wrong, emphasising wrong punctuation

I notice this the odd time, watching random TV shows or films. Actors seemingly reading lines wrong, pausing in the wrong places, emphasising the wrong words...

Tonight it was 'Law & Order: LA' one of the lawyers questioning a defendant asked "what if, anything changed when you came out of prison?"

whereas, I immediately thought: 'that should have been "what - if anything - changed when you came out of prison?" '...

So, is it just me, or have you noticed it happen? can you remember any especially jarring occurrences?
