Illness Benefit - Payment From Job

    Hi
    Due to be off work for approx 8 weeks (maybe more)
    Job is paying me for 8 weeks but if it goes over, then no payment.
    I'm not sure how i go about illness benefit as never been in this situation before.
    I will need to get a cert for the time off which is fine
    But how do i go about illness benefit? Does the doctor / hospital arrange this?
    I won't be able to get to the social welfare office for at least 3 weeks into this time off.
    Does the illness benefit be made out to the job as they will be paying me for the time?

    Any advice would be much appreciated!

    At the start you get the illness benefit application form and fill it out (this only has to be done once, at the start) and your GP will complete a one page form that is only available to GPs. You stick both forms in an envelope and send it off to illness benefit.
    Your GP will indicate on the form when he:she feels you will be fit for work again. If that time comes and your not fit for work then he:she will fill out another form with a new date on it.
    On the form you will complete there is an option for you to have it paid to your employer. You will have to discuss this with your employer beforehand.

    Thank you so much for this - do i get the form myself from my GP (just asking as i won't be able to get to my GP for the first couple of weeks, having surgery that will leave me unable to get there for a couple of weeks) Or can someone get it for me (or can the hospital supply it before i leave!)

    Sorry, a million and one questions :confused:
    Ask your GP to fill up his/her part for you in advance of your surgery. Make sure that the dates are correct as in the exact date you will be unfit for work from until the date he expects you to be fit for work again.
    The form that goes with it you can get it at a SW or Citizens information office and have it filled up and ready.
    Then you can arrange for the forms to be posted off for you the first day you are unfit for work,

    When I had surgery the hospital gave me the form with my expected return to work date. As it happened I had to go to my gp to get my stitches out and he signed me off for an extra week as he wasn't happy with my healing as the surgery had been more serious than was expected.
