Guidance om completing DIY divorce documents.

I've decided to go down the route of a DIY divorce and have obtained the required forms form the nearest Circuit Court (we have them available pre-bundled).
Looking at the documents and while most are self explanatory as how to complete I am not entirely certain on what to put in the "indorsement of claim" part on the Family Law Civil Bill.

Wondering if any of the people here who have gone down the DIY route can provide some guidance.

    Thanks.
