"First Run Whiskey"

Over the years I've often heard people speak of "first run whiskey" - generally obtained from someone who worked in a distillery. Variously described as "rocket fuel", "poison", or "hearty stuff". Only this week, someone described seeing someone else sip it and, apparently, his lips turned blue and started to peel!!

When people talk of this mythical liquid, they can never answer any questions as to what exactly it is other than "first run". No one seems to know what this is.

Does anyone here have any idea of what this sneaked out of a distillery liquid might be?

Couldn't be the first distillation as this would only be around 30%
Unlikely to be the heads or fore shots as these would contain large amounts methanol, taste awful and possibly be very dangerous.
Unlikely to be feints or tails as the ABV would be too low.

Could it just be collection strength, unaged whiskey? This could, technically be up to 94.6%, depending on what was being distilled.

    Any ideas?
And any idea why it's known as "first run"?
