Receipt requirements

    Hi,

    Not sure if this is the correct place to ask this question so Mods please feel free to move if required.

    Anyway, setting up new health care related business (self employed for the time being), and looking for bit of advice what information is required on the receipt to make sure it will be acceptable to customers' insurance.

    Here's what I think so far:
    My name/business name
    Customers' name
    Amount paid
    Date
    Anything else?

    I'll be using those self copying receipt books until I get established and can afford to use custom printed ones.
    Do I need to get company stamp for receipts or my signature is sufficient?
