Where's the cancellation form?

5. Cancellation Process To cancel your order before the expiry of the cooling off period, you may fill out a Cancellation Form and return it to us to let us know of our decision. To meet the cancellation deadline it is sufficient that you send this Cancellation Form or notify us by phone before your cooling off period has expired

Can you let me know where I can find a copy of this cancellation form, as I haven't been able to get through on the phone?

Thanks Linzi,  that doesn't quite look like what I need (as I'm cancelling within the cooling off period, before service has started), but I'll go with something along those lines.
