"Form building" application

Hi, I'm looking for guidance/suggestions for a "cloud form building" route to capture field engineers work? Only two field engineers at the moment.

I'm looking for suggestions on a suitable cloud form builder package that will allow me to setup custom specifications with task(s) that must be completed by a field engineer in setting up and configuring an "equipment build" for customers on their site.

I also need the ability to ability to scan barcodes serial numbers that will identify parts used in the build as well as capturing a customer signature.
A nice to have would be the ability to capture photos at various stages of the build.

Any suggestions appreciated.

Take a look at airtable, has both photo capture and barcode scanning.
