Irish driver's licence renewal - some questions: time frame/forms required?

I have an old style pink Irish drivers licence.
    Standard car "B" and "W" licence - I'm in my 40's.
    It expires next July (2019).

I picked up the application form, guidance doc and medical/eyesight forms for the new type licence at my local office recently.
I've found the guidance notes a bit confusing and vague.

I have some questions - maybe someone has already gone through the process recently.

    1. Can I renew more than 3 months before the old licence expires - or do I have to be within this window?
    2. Is the Medical Report required in all cases, or just for older folks. If so, would a GP do this?
    3. Is the Eyesight report required in all cases? If so, would a GP do this also, or do you have to go to your optician, or Specsavers etc.

    Not before 3 months.

Only if you answered yes to any of the questions in part 5 of the form from 22 onwards

Only if you required glasses in the past and no longer required them e.g. you have had laser corrective surgery or similar.

    Thanks for the replies.

Doe anyone know, if I were to "misplace" my licence, would the replacement only run to the next renewal date (i.e. July) or would I get the full 10 years?
