Does this setup need a headset worn on ear? you can hear them through the radio speakers but how do they hear you?

Inbuilt microphone. No need for a earpiece.

No, it’s got a built in microphone. Note that lots of ford radios have the buttons for phone without actually having the Bluetooth kit installed.

The car already has an inbuilt microphone to pick up speech during a phonecall. There is no need for a earpiece of any sort. People wearing earpieces in a car do so for two reasons A the car has no Bluetooth and the earpiece is connected to the phone or B they do not want the passenger to here the full of a conversation think Taxi driver.

Yep, if you fit the Bluetooth module then they can control the phone. I think the rule of thumb was that those fitted with factory Bluetooth had a voice command button on the steering wheel radio controls.
