Medical card form

    Hi All,


    Just a quick question.


    I am filling in the medical card form.


    In terms of maintenance from child's father I am unsure of what to put down.


    Child's father pays the mortgage on the house.
    Does this count as maintenance and do I need to put a money value to it?



    And in the Mortgage/Rent section what do I then put here?

    It's really your call.

    If you put the amount in under "Any other income" in Part 3, then you'll need to provide "Relevant documentary evidence" - but you probably don't have any - unless you have a written agreement from him that he will do this?

    And if you do that, then you'd need to put in the monthly mortgage cost in Part 4 A and provide mortgage statements.

    If I was you, I'd write "see enclosed letter" in Parts 3 and 4A and enclose a brief letter explaining that he pays the mortgage every month in lieu of maintenance. State the amount of the monthly payment and, if possible, enclose a recent mortgage statement showing the payments that he has made.

    If you're turned down, be certain to appeal within the 21 days and ideally go to your nearest Citizens Information Office for help with the appeal.
