    Hello,

    I wonder if anyone could help me. My ex partner wants to claim this credit for 2014 and 2015, that's fine with me as I didn't use the credits. However could anyone let me know if I have to fill in and send off two separate forms for each year, as it only has space to fill in one year on there? As in 'Tax year for which
    you are making a claim' and then there's a space for one year, if that makes sense?

    If you look at the wording of Section B on the form, you'll see that it says

    "I understand that once I relinquish my entitlement to this tax credit and it is claimed by the individual named below, he or she will continue to claim it until such time as I cancel my relinquishment."

    The Form Notes also say: "Once you have relinquished your entitlement to this tax credit and it is claimed by the secondary claimant, he or she will continue to be entitled to claim it until you cancel the relinquishment."

    So you only need to send it in once - but be sure to input 2014 in Section A of the form.
