Bath mixer taps faulty?

Hi all, Had a problem in main bathroom and had to have the bath mixer taps replaced. Water is coming out of both the shower head and the spout. I've discovered this morning that the hot and cold pipes have been reversed. Would this cause this type of problem or is the mixer tap faulty? Thanks in advance.
