Is this an error code which has appeared to existing equipment without any changes in equipment/use/envronment?

Have you checked for mechanical faults, sounds like an overload.

Less likely is a bad connection on a phase supply to the motor.

This means that your current has exceeded 200% of what the load current should be. This may be due to a short circuit, so carry out an insulation resistance test on the motor and on the cabling. While you are at it it would be no harm to measure the resistance of each winding.

This may also be due to a change in load conditions. Has the load increased? This could also be caused by mechanical fault such as bearing failure so check that the shaft can rotate freely.

Appeared on existing equipment but as you suggest we haven't determined if there are changes to use. Finding it difficult to get to the root of possible changes.
