2006 Mondeo over revving

    Hi All.
Hope someone out there can shed some light on this. I have a 2006 diesel mondeo and this evening when I was reversing in the drive, the engine was running a little bit rough m, so I revved the engine up to about 3k and the engine just revved up to the max. Black smoke billowed from the exhaust. I had to turn the car off to get it to stop. I started the car again and it seemed fine.

Any one any ideas as to why this happened? It did happen before a few years ago too but was fine and I had no repeat instances until today.
