    When do I send Med 1 Form - before or after sending Form 12? When is the due date?

    A physical med 1 form isn't required. There was a health expenses section on the form 12 to complete which would have covered it.

    It does say on the form 12 that a Med 1 must accompany the Form 12 if claiming health exps
