    I wear glasses full time and its on my full car license. Im wondering now when I apply for a motorcycle learner permit will i need to have a completed eye test form

    No sir.

    As long as you continue to declare that you need glasses, you will continue to receive the '01' on your licence/ permit. The only way you'd ever need one again is if maybe you got laser surgery and wished to remove the '01' notation.
