Inheritance Question

My & Sister & I purchased a house jointly many moons ago. Fast forward 15 odd years & we are both married & have our own kids.
What steps do we need to take to ensure our "half" of the house is left to our own next of kin, i.e. if my sister was to pass away that her 50% stake becomes the property of her family?

    Thanks

Do you hold the house as joint tenants or as tenants in common?

If you don't know the answer to this question, go and see a lawyer, because the answer to this question crucially matters and you cannot afford to get it wrong.
