This is unusual. You actually got a letter awarding you DA until July 2018?

In 15 years of reading 100s of DA letters I’ve never seen one with a time limit on it.
Are you fit to go back on JSA?

Not possible to be given a time limit for DA. It's obviously illness benefit you're on.

Yes that’s the standard DA application form.
The GP had to say that you would be unfit for work for at least 12 months.
That doesn’t mean that you would only get it for 12 months.
Have you still got the letter from DA telling you that your application was successful ?

the application was successful have being claiming the DA since july 17 , seen as the doctor said 12 months im wondering what happens at the end of the 12 months

If the letter from DA didn’t say that you would only get the DA for 12 months then you will keep getting it until you write to them and say that you don’t want it any more.
You seemed to think that you got a letter from DA saying that you would only get it for 12 months?
