Squatters Rights

I read this legal definition of Squatter Rights

The doctrine of adverse possession, allows third parties claim to a right over land (to which they originally had no legal title to) which they have occupied continuously for over 12 years with the intention of excluding all others including the true owner (also commonly known as “Squatters Rights”).

I understand the exclusion of the 'true owner' but what exaclty does it mean by 'excluding all others' ? Does it mean no other 'third party' must have shown any claim to the land during that time also ?

AP claims have been defeated by the owner 'looking over the fence'.

By excluding it means treating it as your own property, for example fencing off an extra bit of garden. However the case law is pretty unsettled (IMHO).

Right, so in essence AP is very hard to prove, the burden being on the person seeking AP ?
