Hip arthroscopy Private health insurance

Does anyone know if hip arthroscopy is covered by Laya health insurance? On the cover I have there are specific codes that are covered but I dont see 3654 or 3658 covered. If I'm not covered, does anyone know ballpark figure of cost of procedure? Thanks in advance.
