Apologies if this has been answered already.

If you need corrective lenses, do you need to fill out the form relating to eyesight (with associated eyetest) for renewal of a driving license?

No you don't. Unless your eyesight has improved to the extent that you now claim that you do not require glasses when driving in which case you supply the result of an eye test which certifies this and they will remove the qualifier code '01' from your licence.

From p.3 of the guidance notes....

An eyesight report form is needed where you previously wore glasses/lenses for driving and are no longer required to do so

Last time I applied, I just ticked the box for wearing glasses, and they returned it to me telling me to get an eye test. That was a few years back, so it might have changed since then.

My licence was renewed two and a half years ago and I didn't need to get another test

Were you wearing glasses when you applied for the licence for the first time? You only need a new eyesight report if your circumstances have changed.
