iPhone unlock & screen replacement

So I have an iPhone 5, hasn't been used in a good while, want to give it to my daughter, needs a new screen but also needs to be network unlocked, we go through a lot of phones in our house, no clue which network it's locked to or the number, any recommendations for someone local to do a proper unlock & screen replacement…
