Corporate Manslaughter

With many (failed) attempts over the last 16 years to bring in statutory corporate manslaighter I'm wondering has there been any (successful or otherwise) case law on the issue in *recent years in this jurisdiction.

    * I know there have been no prosecutions between the formation of the state until 2005, but between then and now I'm unsure?

Also, the UK "identification doctrine" has also been approved here, but has it ever been applied for a successful prosecution?

Finally does anyone know what became of the Corporate Manslaughter Bill 2016, has it failed to be implemented just like the many similar bills over the years before it?
