Tax self certification

Hi , Just looking some rough advice on the following. Irish resident , moved to the UK in 2014. Still have my Bank Account in Ireland and of course one in the UK I got a letter from the bank in regards tax Tax residency self certification form which they need filled in. Anyone else had to fill one in?

Hi ,
