    My Mam has been diagnosed with Leukaemia and is going to be in hospital for a good while.
    She's been told about forms for claiming sick leave but not where to find them or what she needs.
    She's already got a letter from her GP and her Consultant is more than happy to sign stuff for her.
    If anyone could help me find out what I need and where to get them I would really appreciate it.

    Illness benefit: This is based on someone's PRSI contributions.

    Disability Allowance: This is a means-tested payment which isn't based on PRSI.

    Sorry to hear about your Mam's illness. She should get Disability Allowance but it will take around 9 months to come through. She can apply for Illness Benefit first but it is dependant on her PRSI contributions. If she has the right ones, she will have it in a week or 2 maybe. If she doesn't, she can apply for Supplementary Welfare Allowance while waiting for the Disability Allowance.

    You can visit your/her local Social Welfare office for advice and they should have the forms there. The GP should have the forms for Illness Benefit there, ask the Secretary. They will fill out their part, there is a box to tick if someone is an inpatient in hospital. Your mam can fill in her part and you can drop it in to her SW office for her.

    You can download the forms also from the DSP website and there's also a facility to request forms to be sent to you. I can't post links on the app so this won't work if you click on it, just type it in to your browser with no spaces.

    Click on Online Services and Forms. Then Application Forms, then Disability and Illness.

    Or Online Services and Forms. On the right hand side under Frequent Online Services, then Other Services, select request an application form.

    You will need your Mam's PPS number.
