Asked for login details when filling in online holiday form

    I just tried to fill in my holiday application form and a login box appeared, I can't remember the last time if I registered for the last application around the start of June, there is no email telling about this nor is there login details saved in my computer or Chrome, where do I find out about my login details for the online holiday application form.
